We ask health and care professionals in Wedmore and Axbridge to think about applying to the Fund if they are in contact with someone who could benefit from the Fund’s support.

To request support, the health or care professional should contact us explaining:

The circumstances and reason for the request.

The cost of the service or equipment, and the duration for which it is needed, if this is known.

Please do NOT include any personal details, including the person’s name or address.
There are no deadlines for applications.
Once the application has been received, the Trustees will assess the request on its merit and allocate funds if appropriate.
